Health Forms

At Travis Unified, the health and safety of our students is a top priority. To provide the best care and support for our students, we require parents and guardians to complete several important medical authorization forms. These forms are crucial for ensuring that your child receives appropriate care at school and during school-related activities.

Medication Authorization Form

  • Purpose: This form authorizes the school to administer prescribed medication to your child during school hours.
  • Requirements: It must be filled out and signed by the child’s healthcare provider and also signed by the child's parent or guardian to authorize school staff to administer medication during school hours.

 

Health Exam Physical Form

  • Purpose: To ensure that every student is healthy and fit to participate in school activities, especially physical education and sports.
  • Requirements: This form needs to be completed by a licensed healthcare provider and submitted to the school nurse or designated school official.
  • Athletic Physicals need to be submitted to LINK or LINK

 

Oral Health Assessment Form

  • Purpose: Oral health is an essential part of overall health. This form helps us monitor and address any dental concerns that may affect your child’s wellbeing and school performance.
  • Requirements: To be filled out by your child's dental care provider and submitted to the school nurse or designated school official.
