2021-22 Information and Elective Forms

  • Golden West Schedule Change Request Form:

    To request a schedule change students should submit the Google Form below by August 17, 2021.
    The deadline for schedule change requests is now CLOSED. Below applications will remain open while seats are available.

    For students who submitted a course change request form, be sure to review below procedures for these requests:


    Priority will be given to the following requests:
    *Students missing periods
    *Students will duplicate classes/periods
    *Students in the wrong level course

    The following requests are NOT accepted reasons for schedule change and may be denied:
    * Request to have/not have specific teacher
    * Request for specific period

    All other requests will be reviewed per seats available. Previously submitted student elective ranking forms will be reviewed.

    Please know that class changes are not guaranteed. Students should fully participate in their provided schedules and will be notified if a change is available.

    This form will be available through Tuesday, August 17 All schedule requests must be submitted by that time. Counselors will be reviewing submitted requests through Tuesday, August 24 and will notify families as changes are made.

    *Important: Please remember that requests will be prioritized to ensure that all students are placed in a full schedule. To ensure requests are able to be handled in a timely fashion, we request that schedule requests be directed through this form only. This form should be submitted directly by the student; parent/guardian consent is required for consideration.
